This Telehealth Informed Consent does not modify or supersede any Terms of Use, Privacy Policy, or Notice of Privacy Practices of Medical Media Partners LLC – DBA Overnight Weight Loss or the Providers, but rather supplements those documents.

  6. Agree that, in compliance with law, some of your care may be delivered asynchronously via store-and-forward technology through the Medical Media Partners LLC – DBA Overnight Weight Loss Platform rather than real-time.

  7. Understand there is no guarantee of prescription and that prescribing decisions rest in your Provider’s professional judgment.

  8. Acknowledge the risk of technical failures and agree to hold harmless Medical Media Partners LLC – DBA Overnight Weight Loss and its affiliates (including their employees, contractors, directors, officers, agents, predecessors, and successors) for delays or information loss due to such failures.

  9. Understand that third-party diagnostic tests (labs) ordered via the Platform may be defective or erroneous, potentially impacting the quality or accuracy of care.

  12. Know you have the right to withdraw consent for telehealth at any time without prejudice, though Providers using the Platform do not offer in-person alternatives.

  13. Acknowledge you may access your medical records through the Platform under applicable law, and may authorize your primary care provider to obtain copies.

  14. Understand you may choose any pharmacy or laboratory for services, regardless of those offered via the Platform.

  15. Agree that Medical Media Partners LLC – DBA Overnight Weight Loss is a third-party beneficiary of this Telehealth Informed Consent and has the right to enforce it.
